Testimonial and Photo Release: In consideration of good and valuable consideration, the receipt and sufficiency of which is hereby acknowledged, I, the undersigned, hereby grant to Vixen ready art factory LLC (the “ART STUDIO”) and their agents (NORTHLAKE MALL), organizers and representatives, the right to use my name, biographical information, photographs, images, story and/or testimonial, in whole or in part, and without restriction as to changes or alterations. The rights granted herein shall extend in perpetuity, unless revoked in writing to the ART STUDIO by me, throughout the world and for any purpose whatsoever, including without limitation for marketing and advertising purposes of the ART STUDIO and its agents, and in any and all media, including without limitation the ART STUDIO’S website and social media outlets. I acknowledge that the ART STUDIO and its agents have no obligation to return any photographs or images to me. I hereby RELEASE, WAIVE and FOREVER DISCHARGE any and all claims arising out of, or in connection with, such use by the ART STUDIO and its agents, including without limitation any and all claims for libel or invasion of privacy. I hereby warrant and represent that I am at least 18 years of age and have the right to contract in my own name (if under 18, I have the right to represent all children under 18 during our visit to the ART STUDIO and waive all/any obligations to the ART STUDIO. I have read the above Release and am fully familiar with the contents thereof. This Release contains the entire agreement between the parties hereto as to the subject matter contained herein.

I have read this Testimonial and Release Form, fully understanding its terms, that I give up substantial rights by signing (printing my name) and sign it voluntarily.

Personal Responsibility with Alcohol: Consumption of any alcohol at events is optional and a personal decision. By running with (Vixen Ready Art Factory LLC, I agree that any alcoholic beverages I consume are my own choice and responsibility. I agree to drink responsibility and heed suggestions from others and will not drive if I am intoxicated. I acknowledge that I alone am responsible for all of my actions resulting from consuming alcohol and the ART STUDIO is in no way responsible.

Waiver: I agree that I am a customer of the ART STUDIO, and I know that upon entering in the ART STUDIO and participating in any ART STUDIO activities, In-studio activities and off-studio events!

I attest that I have read and agree to abide by the rules of the ART STUDIO. I assume all risks associated with being a customer of this ART STUDIO and participating in ART STUDIO activities which may include: PAINTING, WINE TASTING, CANDLE MAKING (with hot wax), the usage of art supplies (such as scissors, staplers or any of arts and crafts tools, along with any legal consumption of alcoholic beverages (Adults 21 and older); and contact with other participants during COVID or any PANDEMIC EMERGENCIES! We are not responsible for any medical illnesses due to contact within the ART STUDIO.

Having read this waiver and knowing these facts and in consideration of you being a patron of the ART STUDIO, I, for myself and anyone entitled to act on my behalf, waive and release this ART STUDIO , the ART STUDIO organizers, the NORTHLAKE MALL , and all ART STUDIO sponsors, their agents, representatives and successors from all claims or liabilities of any kind arising out of my participation with the ART STUDIO , even though that liability may arise out of negligence or carelessness on the part of the persons named in this waiver.
